Job Summary: The BBBL Grant Pharmacist is a split position primarily consisting of the clinical duties of coordinating concise inpatient care of COPD patients. Duties include medication reconciliation, medication therapy management in coordination with hospitalists, smoking cessation counseling, and coordination of discharge services. Developing a close professional relationship with patients while they are here in the hospital is critical to assist with improving adherence, inhaler technique, understanding of medication regimen, and to facilitate follow up after discharge. Cary Medical Center was awarded the BBBL Grant and was the hospital of origin for the program, which is being phased into 4 other hospitals (MDI, NMMC, Houlton, and Millinocket). The BBBL Pharmacist will also coordinate care for the COPD patients meeting the grant's criteria in these hospitals utilizing Zoom or other video calls for patient communications. BBBL Grant duties will comprise about 30 hours of the work week.
The other approximate 10 hours per work week will be functioning as a Staff Pharmacist. Staff Pharmacists fill orders for medications, monitor patient medication therapies, and provide drug information. Pharmacists supervise and direct support personnel. In the absence of the Director of Pharmacy, a Pharmacist may be required to assume the essential responsibilities and perform the duties of the Director of Pharmacy.
Education, Skills, Experience Requirements:
Graduate of an ACPE-accredited school of Pharmacy with a BS or Doctor of Pharmacy degree.
Active pharmacist license awarded by the State of Maine.
Health-system experience preferred, but not required.
Ability to work independently with minimal direct supervision.
Ability to work cooperatively with health-system and pharmacy staff.
Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
Ability to set priorities makes critical decisions, and responds quickly to emergency requests. Ability to exercise sound professional judgment.
Ability to communicate effectively (orally and in writing). Ensures pharmacy services are provided in a timely manner.
